Nurse Case Manager - Clinical Care -

Job Description:

Immediate need for RN Case Manager to conduct assessments, care plans and make medical determinations of services requiring prior authorization.This will include short and long term catastrophic and chronic needs.Communicate with family members,develop individualized care plans,provide clinical and liaison services that include follow-up of discharge from the ER, Acute Care and inpatient admissions.

Requirements: are an active California license as a Registered Nurse and two years clinical nursing experience.Bachelor?s degree in nursing or a related health or social services field is a plus.Certification as CCM or equivalent is preferred.Geriatric experience and familiarity with community resources helpful. Strong working knowledge of case management, Medicare, and Medi-Cal benefits, as well as the complexities of working with the elderly and disabled populations.Experience utilizing personal computer and with Microsoft Office products (Word, Excel, Access and Outlook). Bilingual skills (Spanish, Tagalog or Chinese) highly preferred.
