acute inpatient case manager experience required License: Valid RN
License Certifications: BLS (American Heart Association) Must-Have:
Epic experience preferred Job Description: - Responsible for care
coordination and care transitions planning throughout the acute
care patient experience. - Collaborate with physicians, utilization
managers, medical social workers, and bedside RNs to ensure timely
progression and transition of patients to appropriate levels of
care. - Facilitate communication and care across the continuum
through effective resource coordination to prevent unnecessary
admissions or readmissions. - Achieve optimal health, access to
care, and appropriate utilization of resources balanced with
patients' self-determination. - Conduct initial and ongoing reviews
of physician admission care plans, gathering medical, psychosocial,
and financial information. - Screen for ancillary supportive
services, including Palliative Care Services needs. - Lead the
healthcare team in developing comprehensive, cost-effective care
coordination plans. - Identify and resolve barriers to care
progression and transitions in a timely manner. - Collaborate with
financial counselors for delivery of inpatient stay denials. -
Ensure compliance with state and federal regulatory requirements
for transitions and post-acute transfers. - Participate in patient
rounds and collaborate with the interdisciplinary team for timely
transitions. - Maintain current knowledge of CMS, Joint Commission
transitions of care requirements, and other regulatory standards. -
Communicate effectively with patients, families, and the healthcare
team while addressing complex clinical and social situations. -
Make appropriate care plans and referrals to community resources
for high-utilization patients. - Participate actively and
positively in department operations, fostering a collaborative
environment.
